Survey/Quiz Management

When you would like to create a survey, form or quiz that visitors to your website can fill out, you can use the Survey/Quiz Management option. This option is located under Site Section Administration options after choosing a Site Section from the Admin Home. When you would like to collect information from a user, a Survey/Form is the best option. If you would like to test a user and be able to examine the percentage of questions answered correctly, you will instead want to add a Quiz.

Adding an Online Survey or Form
 
You can click the Add Survey/Form button to begin creating a new Survey or Form.

Type a name for your Survey/Form in the Title box. You can also add a brief Description to that visitors will see.

If you would only like the owner and optional editor of the Survey/Form to be able to view the results, check the Results Private? checkbox. If you check the Anonymous OK? checkbox, a name and email address will not be required for users filling out the Survey/Form.

You can add a Confirm Message to your Survey/Form that users will see after filling it out. If you would like email notifications to be delivered when a user fills out the Survey/Form, you can add the email addresses in the Alert Email(s) box. Multiple emails in this box must be separated by commas only, and this box is also limited to 150 characters.

If you only want your Survey/Form to appear between certain dates, you can use the Date Visible and Date Expires boxes. Once you have finished configuring these options, click Save to create your Survey/Form.
 
Adding Questions to a Survey or Form

Now that your Survey/Form is created, you may add Questions to collect information from users. Questions are added to your form in much the same way that Components are added to a Content Page.

Choose the type of question to add with the Type dropdown box. In the Question Text box, add the wording for the question that you would like users to respond to. By checking the Is Req'd checkbox, you can require users to answer the question before submitting the Survey/Form.

You can also add a downloadable file to your Survey/Form, by clicking the button next to Download File. If you would like an image to be displayed with this question, you can upload it with the button next to Visible Image.

Below is a summary of Question Types that you can add to your Survey/Form:
  • Yes/No: A question that has two possible choices for the user to respond with: yes or no.
  • Single Text: Users have a single line to type the answer to the question.
  • Free Text: Users have a large text box to type the answer to the question.
  • Radio List: A multiple-choice question, where users may only select one of the possible answers.
  • Checkbox List: A multiple-choice question, where users may select as many answers as they choose.
  • Document Upload: Allow users to upload documents to your survey form or quiz with this Question, such as pdfs, word documents or images.


When editing or creating a Question that has multiple answers, such as a Radio List or Checkbox List, you will add each answer individually. Type your answer into the Option box, then click Add. Your answer will appear listed below. You can remove an answer to this Question by clicking the Delete button.






Once you have finished creating your Question, click the Save Question button. Your question will appear listed below Existing Questions. Here you can Edit or Delete your Questions. You may also reorganize the order of the questions by clicking and dragging on the green arrows along the side of the Question.

After saving any changes to your Questions, you may safely exit the page by using the Breadcrumb Navigation at the top of the page.



 
Adding an Online Quiz
You can click the Add Quiz button to begin creating a new Survey or Form.

Type a name for your Quiz in the Title box. You can also add a brief Description to that visitors will see.

If you would only like the owner and optional editor of the Quiz to be able to view the results, check the Results Private? checkbox. If you check the Anonymous OK? checkbox, a name and email address will not be required for users filling out the Quiz.

Set a number for the Passing Grade to determine what percentage of questions a user must answer correctly to pass the Quiz. You can add a Passing Message and Fail Message to your Quiz that users will see after completing the quiz with a passing or failing grade respectively. You can also add a Passing URL and Fail URL, which will create a link that will appear for the user upon passing or failing the quiz respectively. 

If you would like email notifications to be delivered when a user fills out the Quiz, you can add the email addresses in the Alert Email(s) box. Multiple emails in this box must be separated by commas only, and this box is also limited to 150 characters.

Adding Questions to a Quiz

Now that your Quiz is created, you may add Questions that users must answer. Questions are added to your Quiz in much the same way that Components are added to a Content Page.

Your Questions are added to the form one at a time. Begin by typing in the Question Text, which is the question your users will try to answer. Options are the possible answers that your users will be able to select from for this Question. Type the text for your Option, if it is the correct answer check the Correct? checkbox, then click Add to add your Option below. You can delete existing Options or change which Option is correct with the Delete and Correct buttons.

By checking the Is Req'd checkbox, you can require users to answer the question before submitting the Quiz. You can also add a downloadable file to your Quiz such as a pdf by clicking the button next to Download File. If you would like an image to be displayed with this question, you can upload it with the button next to Visible Image.

Once you have finished creating your Question, click the Save Question button. Your question will appear listed below Existing Questions. Here you can Edit or Delete your Questions. Showing a Survey, Form or Quiz on a Content Page
 
After creating a Survey, Form or Quiz, you can now display it on your site for users to fill out. Edit a Content Page on the same site section where you created your Survey/Form/Quiz. Add a new page component, using a Survey/Form Component if you would like to display a Survey/Form, or a Quiz component if you would like to display a Quiz.

Now edit the component and select the Survey, Form or Quiz you want to display. Click Save and your Survey, Form or Quiz will now appear on this Content Page.
 
Managing An Existing Survey, Form or Quiz and Viewing Responses

You can see any any existing Surveys, Forms or Quizzes that you have access to. These can be viewed in Survey/Quiz Management, below the Add Survey/Form/Quiz buttons.


Click the Edit button to make changes to Survey/Form/Quiz. You can use the Copy button to make a duplicate of the Survey/Form/Quiz. To view the responses users have submitted for a Survey/Form/Quiz, click the View button.

You can choose how to view the responses using the Show Report dropdown. The Question Aggregate Summary combines all responses to show the frequency that users answered questions a certain way. Please note that only multiple-choice questions can be aggregated. The Individual Responses view allows you see each Survey/Form/Quiz that was submitted.
